 As we transition to permanent locations, our focus will be on establishing two key facilities that will serve as the cornerstone of our long-term operations. These facilities will provide a stable environment for more intensive interventions and support services. 

Phase 3: Establishment of Physical Facilities

Step 1: Gain Access to Property

 

Our facility selection process involves thorough research, site visits, and consultations with local authorities and community stakeholders. We prioritize proximity to public transportation, healthcare providers, and essential services to ensure our clients have the best possible setting for their rehabilitation journey. This collaborative approach helps us identify locations that not only meet our operational needs but also integrate well with the surrounding community.

Patrick R Foundation Corporation is committed to meeting or exceeding all regulatory requirements for healthcare and housing facilities. We are actively working with local authorities, including zoning boards, health departments, and social service agencies, to secure necessary permits and certifications. This proactive engagement demonstrates our dedication to compliance, quality care, and community integration, while also helping us anticipate and address potential challenges in the facility establishment process.

Court-Mandated Programs and Case Management Facility

We will accomplish this by either adjoining the halfway house or by finding a separate off-site location.  This will be determined by zoning regulations and space availability. 

 

  • Features:
    • Multiple group therapy rooms of varying sizes to accommodate different types of sessions and group dynamics.
    • Private offices for individual counseling and case management, ensuring confidentiality and personalized attention.
    • Welcoming reception area and comfortable waiting room, designed to reduce anxiety and create a positive first impression.
    • Secure storage for client records and essential items, maintaining privacy and compliance with data protection regulations.
    • State-of-the-art technology infrastructure, including telehealth capabilities for remote consultations and digital record-keeping systems.

Establish a Halfway House

 

  • Initial capacity: 20 beds, with potential for future expansion based on community needs and available resources.
  • Location: Carefully selected site in the Capital Region, balancing accessibility, safety, and a supportive environment. We will consider factors such as proximity to public transportation, healthcare facilities, and potential employment opportunities.
  • Amenities:
    • Private and semi-private living quarters, designed to provide a sense of dignity and personal space for residents.
    • Common areas for group activities and socialization, fostering a sense of community and mutual support among residents.
    • On-site medical and counseling spaces, ensuring easy access to essential health services.
    • Communal kitchen and dining area, promoting nutrition education and community-building through shared meals.
    • Modern shower and laundry facilities, maintaining the hygiene standards established in our mobile services.
    • Outdoor recreational space (where possible), providing opportunities for physical activity and stress relief.

Vehicles

 

To enhance our ability to serve clients effectively and extend our reach beyond our physical facilities, we will acquire two essential vehicles:

  1. A dedicated car for transporting clients to medical appointments, job interviews, and other crucial engagements. This vehicle will be equipped with GPS and scheduling software to optimize routes and ensure timely arrivals.
  2. A van equipped with mobile showers to meet the immediate hygiene needs of the homeless population and serve as a first point of contact for building trust within the community. This van will also carry essential supplies such as toiletries, clean clothing, and information about our services.
